본 연구는 기저귀 종류에 따른 가정 경제부담과 환경에 대한 인식, 그리고 피부자극도를 비교조사 연구이다. 본 연구의 대상은 서울시내 2개 종합병원의 영유아 클리닉을 방문하는 기저귀를 사용하는 영유아를 가진 어머니 208명으로서 본 연구의 목적에 맞게 개발된 설문지를 사용하여 자료를 수집하였다. 피부자극도 검사를 위한 연구대상은 20대의 건강한 성인 30명으로 이들의 피부에 자극물질을 처리한 후 피부변화를 관찰하였고 corneometer를 이용하여 각질층 수분함량을 측정하였고, 증발계를 이용하여 TEWL을 측정하였다. 자료분석은 SAS 통계프로그램을 이용하여 전산처리하였다. 본 연구의 결과는 다음과 같다. 1. 대상자의 일반적 특성에서 나타난 바에 의하면 기저귀 사용종류는 종이 기저귀 사용자 52.9%, 병용형 24.3%, 천기저귀가 22.8%로 나타났다. 2. 기저귀 선택이유는 천기저귀의 경우 피부보호가 가장 높았으며, 종이기저귀와 병용형은 편리성이 가장 높았다. 3. 기저귀 종류별 사용갯수와 구매비용의 비교에서 하루동안 기저귀 교환횟수는 병용형이 가장 많았고 종이기저귀 사용자가 가장 적었으며 기저귀 종류집단간에 통계적으로 유의한 차이가 있었다(F=55.3, P<0.001). 구매비용에서 천기저귀가 가장 비용이 적었고 기저귀 사용집단간에 통계적으로 유의한 차이가 있었다(F=6.7, P<0.001). 4. 대상자의 환경관련 지식과 인식은 세 집단간에 통계적으로 유의한 차이가 나타나지 않았고 모두 낮은 수준이었다. 5. 피부질환별 발생경험은 기저귀 종류별로 분석한 결과 종이기저귀 사용자가 피부질환 경험을 많이 한 것으로 나타났다. 대상자들이 주로 많이 경험한 피부질환은 아토피 피부염(36.24%), 기저귀 발진(35.81%), 땀띠(25.76) 순이었다. 6. 기저귀 소재별 자극도를 실험한 결과, 천기저귀를 사용한 경우에서 일시적으로 TEWL의 증가와, 각질층 수분함량의 감소를 보였으나, 지속적인 유의한 차이는 볼 수 없었다. 이상의 연구결과를 토대로 다음과 같은 제언을 하고자 한다. 1. 기저귀 사용과 관련되어 대상자들의 환경에 대한 지식과 인식정도가 낮은 것으로 본 연구결과에서 나타났으므로 기저귀를 사용하는 영유아를 양육하는 부모나 보호자들을 대상으로 한 환경보호에 대한 교육 및 홍보가 절실하다. 2. 본 연구의 피부자극도 실험에서 표본의 크기가 충분하지 못하여 일회용 종이기저귀가 피부자극을 줄일 수 있다고 연구결과를 일반화하기 어려우므로 대상자의 수를 증가하여 기저귀 종류에 따른 피부자극도를 재확인하는 추후연구를 제안한다. 또한 기저귀 종류에 따른 피부의 자극도 검사를 영아에게 직접 적용하여 확인하는 실험연구도 제안한다.

Although the use of disposable diaper is increasing recently, various factors should be considered to determine which diapering system may be used. This study was performed to investigate the effects of cloth and disposable diapers on home economy and environment and the irritancy to the skin. Questionnaires were given to 206 mothers visiting well baby clinics at Severance hospital and Samsung Jell hospital. To investigate the irritancy, we observed skin changes in healthy volunteers of 3049 years old before and after applying sodium lauryl sulfate(SLS) or ammonia absorbed cloth and disposable diapers. The irritancy was evaluated by using corneometer and evaporimeter and clinical changes of the dependent area of the skin.
Among 206 babies, 109 babies(52.9%) used disposable diaper, 47 babies(22.8%) used loth diaper, and 50 babies(24.3%) used both. The important considerations for choosing diaper type were the convenience(38.1%), safety of skin(21.8%) and environmental aspect(19.4%). But only 9.3% thought about the expenses. The average frequencies of changing diapers were 8.4±3.8/day in groups using disposable diaper, 16.8±8.4/day in groups using cloth diaper and 18.3±8.0/day in groups using both diapers in combination. The cost for diaper per month was lower in cloth diaper(w36,214±31,645) than in disposable diaper(w44,090±26,518) and in combination(w67,106±71,355). The knowledge and perception about environmental impact of the diaper use was low in all groups. There was no statistically significant difference between groups.
Sixty five percent of babies using disposable diapers experienced skin problems, 44.7% of babies using cloth diapers, and 17.3% of babies using both types of diapers. Common skin problems they had were atopic dermatitis, diaper dermatitis, and miliaria. After applying irritant(SLS/5% ammonia)absorbed diaper patch on the skin of healthy adult volunteers, the transient increase of
transepidermal water loss and decrease of water content in stratum corneum were observed in the skin of volunteers applying cloth diaper patch, but these changes returned to normal after 1 hour.
Differences in clinical changes of the skin were not observed between two volunteer groups applying cloth and disposable diaper patch.
